- Collect Payment Receipt: Take your payment receipt to the UDUTH School of Midwifery cashier. You will receive a cashier’s receipt, serial number, and PIN.
- Fill the Application Form: Visit the UDUTH Basic Midwifery application page and enter your serial number and PIN to access the form. Fill in your details accurately and submit the form online.
