- Professional Certificate: Registered Nurse (RN)
- Educational Certificates: FSLC, WASC, SSCE, GCE, NECO, or NABTEB with five (5) credit passes, including:
- English Language
- Mathematics
- Physics
- Chemistry
- Biology
(Credits must be obtained in not more than two sittings)
- Testimonial from the last professional school attended
- Birth Certificate or Age Declaration
- Change of Name document (if applicable)
- Certificate of Origin
- Two recent passport photographs with red background
