Who Can Apply?
For National Diploma (ND) Applicants:
To qualify for the ND programme, you must:
- Sit for the 2025 UTME
- Meet or exceed the UTME cut-off mark for Polytechnics/Monotechnics (as set by JAMB)
- Have five O’Level credits (WAEC/NECO/NABTEB in no more than two sittings) in:
- English Language
- Mathematics
- Biology
- Chemistry
- Physics
- Choose SHIM, OAUTHC as your first or second choice, and be willing to switch to first choice if admitted
For Higher National Diploma (HND) Applicants:
To be eligible for the HND programme, candidates must:
- Meet the same O’Level subject requirements as ND
- Hold a National Diploma in Health Information Management with at least Lower Credit
- Have completed a one-year industrial training (IT) in a recognized health facility
- Possess a valid professional license
Entrance Exams & Screening Details
ND Applicants Will Be Tested In:
- English Language
- Mathematics
- Biology
- Chemistry
- Physics
- General Paper
Format: Written and oral interview sessions
HND Applicants Will Be Tested In:
- English Language
- Professional courses in Health Information
- General Paper

Application Fees:
- ND: ₦2,000
- HND: ₦8,000
Payment Method:
Pay through REMITA to OAUTHC, Ile-Ife.
After Payment:
Visit the SHIM office to collect your application form at:

Deadline for Submission:
All completed forms must be returned by Tuesday, 23rd September 2025.
